Sciatica is a term used for pain and other symptoms caused by irritation or compression of a nerve in the lower back. It often affects one leg and may be felt in the buttock, thigh, calf or foot.

It can be alarming, especially when the pain is sharp, burning or travels below the knee. The good news is that many cases improve with the right advice, steady movement and a plan that matches the person in front of us. At Move Free Chiropractic Pontypool, the first job is not to rush into treatment. It is to understand your symptoms, rule out anything more serious and work out what is likely to help.

What does sciatica feel like?

Sciatica is different from a local ache in the lower back. Typical symptoms can include:

  1. Pain travelling from the lower back or buttocks into one leg

  2. Burning, electric-shock or shooting pain

  3. Pins and needles or altered sensation in the leg or foot

  4. Numbness

  5. A feeling of weakness or heaviness in the leg

Not everyone with sciatica has back pain. For some people, the leg pain is the main problem.

What causes sciatica?

The sciatic nerve is the largest nerve in the body. It is made up of nerve roots that leave the lower spine and travel through the buttocks and down the leg. Sciatica happens when one or more of these nerve roots become irritated or compressed.

Common reasons include a disc bulge or disc herniation, age-related narrowing around a nerve, or irritation linked with a sudden change in load, such as lifting, long periods of sitting, a new training routine or a flare-up of back pain.

A proper assessment looks at your history, movement, strength, sensation and reflexes where appropriate. A scan is not routinely needed for every person with sciatica.

What can help in the early stages?

The aim is usually to keep you moving without repeatedly aggravating symptoms. Complete bed rest is rarely helpful. Short, regular walks and changing position often work better than staying in one posture for hours.

Useful starting points may include:

  1. Breaking up prolonged sitting, driving or standing

  2. Keeping up gentle daily movement within tolerable limits

  3. Adjusting gym, running or workloads temporarily

  4. Using positions or movements that reduce leg symptoms

  5. Taking medication advice from a pharmacist, GP or prescribing clinician when needed

There is no one exercise that is right for everybody. If a movement significantly increases pain travelling down the leg, stop and get it assessed.

How can chiropractic help with sciatica?

Chiropractic care for sciatica should begin with a thorough assessment and an honest conversation about whether it is appropriate for you. If your presentation is suitable for conservative care, your plan may include advice on activity and pacing, targeted movement and strengthening work, and hands-on treatment where appropriate.

Manual therapy is not a magic fix and should not be the only part of the plan. The goal is to help you move with more confidence, gradually rebuild your tolerance and make a sensible return to the things you enjoy.

If your symptoms suggest you need imaging, a medication review, a specialist assessment, or urgent medical care, we will explain why and help point you in the right direction.

When should you get urgent medical help?

Do not wait for a chiropractic appointment if you have new symptoms such as:

  1. Numbness around the genitals, anus or inner thighs

  2. Difficulty starting to pass urine, loss of bladder or bowel control, or not knowing when you need to go

  3. Severe or worsening weakness or numbness in both legs

  4. Sciatica affecting both legs, especially with the symptoms above

These can be signs of a rare but serious condition called cauda equina syndrome. Go to A&E or call 999. Do not drive yourself.

You should also seek prompt medical advice if your pain is rapidly worsening, follows significant trauma, comes with fever or feeling generally unwell, or you have unexplained weight loss or a history of cancer.

When to book an assessment

If leg pain, tingling or numbness is affecting your work, sleep, walking, training or confidence, an assessment can help clarify what is going on and what your next step should be. You do not need to wait until the pain becomes unbearable.
